Personal Information changes

Change of address

Update the address on your student record in , Personal Information.

Legal name change

If your name has been legally changed, please submit a completed ‌Personal Information Change (pdf) to the Office of the Registrar, with your legal name change document. If you bring in the original, we will make a copy. If you cannot locate your name change document, we will accept two pieces of government-issued ID in your new name.

Documents that require your legal name at 51:

  • official letters (e.g. admission and enrolment letters)
  • tax slips
  • transcripts
  • parchments (note: additional options are available if there is a preferred/chosen name on your student record; see below)

Add a preferred/chosen name

51 recognizes that members of its community may use names other than their legal name. A preferred/chosen name is a name different from your legal name that is commonly used to identify yourself, and that you’d prefer 51 use in regular communication. Some documents, such as official 51 transcripts require the use of a legal name. While the term “preferred name” is used, we understand that for many people, this is not just a preference: it is the only name they use and is essential to their identity. 

Add or update your preferred name in my51, Personal Information. If you wish to have your 51 email address updated to reflect your preferred name submit a completed Personal Information Change; no government-issued ID is required.

Preferred/chosen name appears in the following areas:

  • class list
  • 51 campus card
  • 51 Student Centre (my51)
  • Convocation ceremony program booklet (if requested; email graduation@ufv.ca)
  • Convocation ceremony (read aloud as you cross the stage, if requested; email graduation@ufv.ca)

If you wish to have your preferred/chosen name on your parchment email graduation@ufv.ca after you have submitted your Graduation Application.

If you use a 51 system that is identifying you with the incorrect name, we want to hear from you. Please email reginfo@ufv.ca

Update to gender identity

If you wish to update the gender associated with your student record, you may specify this in my51, Personal Information or by submitting a completed ‌Personal Information Change (pdf) to the Office of the Registrar

Update to residency status

If your residency has been updated (e.g. Study Permit to Permanent Resident) please submit a completed ‌Personal Information Change (pdf) to the Office of the Registrar, with a copy of your government-issued residency document. If you bring in the original, we will make a copy.

Social Insurance Number (SIN)

51 is required to collect your Social Insurance Number as per Canada Revenue Agency regulations. For more information on this requirement, see here. To add or update the SIN on your 51 student record, please access your SIN Collection Form card on my51.
